This normally means the part at the back of your neck above the spine and below the base of your skull - if you put your head back you can feel the very top of your spine with your finger, and that is where the hollow begins. But if you are measuring the length of a garment you will normally measure from the front of the neck as this is easier - these images will show you where to start.
