MDB file is the database format created in Msaccess. It is not necessary to create an extension, once you save the file it will store in MDB format.
Yes you can directly created a table in Ms Access

The Office database files have .mdb extension. For e.g if you have saved any file in MS Access, it will automatically be saved with .mdb file extension. And of course, you can directly create a table in MS access.
