Mukthinath Temple is in Nepal. Kathmandu to Pokhara (approx 200 km) by bus. From Pokhara to Jomsom by flight. From Jomsom about 22 km by walk and by truck to Mukthinath. On the way we have to cross Kali River (one truck up to Kali River and another truck from Kali River to Mukthinath). Jomsom to truck stand and from Mukthinath to the Temple we have to walk.

Muktinath also has the local name of Chumig Gyatsa (Hundred Waters).

It's in central Nepal, sandwiched between India and Tibet.  104 km northwest of Kathmandu, on the Annapurna Hiking Trail.  The temple is at an altitude of 3750 m (so high up in the mountains, which is true of most of Nepal!).

Detailed travel information, including how to get to Muktinath, on the temple's official website.

It's an important pilgrimage site for both Hindus and Tibetan Buddhists.  The Buddhists consider it a residence of Goddesses called Dakinis (Sky Dancers).  Hindus travel to the temple because it is connected with several holy Hindu figures.  Most of all the God Vishnu, who turned into stone there are thereabouts.  There are many stories connecting Vishnu to the local Kali Gandaki River, which is just a few hours walk away from Muktinath.
