Depending on what kind of a lab you taking

it should involve the following:

Introduction
Aim for the experiment
Procedures done
Data that been collected
Calculations made and Discussion concerning the results you obtained
and finally Conclusion, usually the conclusion is an answer to the aim of the experiment

It depends on what kind of lab you are taking, but a practical science report should include the following:

Title
Aim (for the experiment)
Background (background information on what you are looking at)
Materials (Apparatus and Materials)
Procedure (Variables and Method)
Risks   (eg. Chemical:  Hydrochloric Acid,
  Dangerous Element: Corrosive
  Risks: Burns
  Risk Management: Safety goggles, apron
Graphs
Discussion
Evaluation
Conclusion (Summary)
Bibliography (Depending on teacher)
