The Hydronium ions H+ and Hydroxyl ions OH- makes acid and bases react with each other and other elements. The above ions form water and the remaining parts of the respective acid and base forms the salt.

Example:
HCl+NaOH---->NaCl+H20

H+ of HCl reacts with OH- of NaOH to form water and Na reacts with Cl to give salt.
